Each year, CNF asks caregivers and clinicians to tell us what support is most needed right now. Your input guides where we focus next.

For Caregivers
• Your answers are anonymous and will directly shape the programs and support CNF provides for families.
• The survey takes about 10 minutes and is open through December 1, 2025.
• If you complete all required questions, you can choose to enter a raffle to receive a SAMi 3 Camera. The winner will be notified by email by December 5, 2025.
